2024-2025 Student Health Requirements

All students entering 9th grade, transfers, and any students entering an Illinois school for the first time are strongly encouraged to have physical and up-to-date immunizations, vision and dental exams by the first day of school. In addition, students entering 12th grade will be required to show proof of recent meningococcal vaccination given at appropriate intervals. Any student not in compliance by October 15th, by state law, will not be allowed to attend school until in compliance. Additional immunization requirements can be found at www.idph.state.il.us.

Vision Exam

Illinois law requires that proof of an eye examination by an optometrist or physician (such as an ophthalmologist) who provides eye examinations be submitted to the school no later than October 15 of the year the child is first enrolled or as required by the school for other children. The examination must be completed within one year prior to the first day of the school year the child enters the Illinois school system for the first time. The parent of any child who is unable to obtain an examination must submit a waiver form to the school.

Medical Waiver & Release Form

The medical waiver and release form is to acknowledge that there are inherent risks associated with participating in school sports and activities. 

Community Service

Students use this form to report Community Service hours. Students should give the completed form to the Registrar to have their hours credited. Alternatively, students can enter Community Service hours directly in Gradelink for approval. Please refer to the Student Handbook for guidance on what counts as Community Service.

Current Grade Request

Use this form to request a copy of a student's current grades. This is the form to use to request grades for car insurance purposes.

Transcript Request

Use this form to request an official high school transcript. Please include a first-class stamp with your transcript request.

IHSA Exam

All athletes and students participating in IHSA activities need to have a current exam on file. This form includes the required IHSA Steroid Testing Policy Consent.

Photo/Media Release

This form grants COS permission to use photos and videos of students in COS marketing, newsletters, news releases, bulletin boards, social media, and the web.

Medication Administration

This form gives COS personnel permission to dispense specific medications to students. The form must be on file before any type of medication can be given to a student.

Motor Vehicle Consent Form

Before volunteers or staff transport students, COS needs this form giving permission to do a driving background check.
